ROBIN J. CONNOLLY APRN, FNP-BC

Robin provides primary medical and health management services to chronically ill, disabled, elderly, homebound and assisted living residents in Rensselaer, Albany, and lower Saratoga county.

Over 35 years of nursing experience in acute and home care settings. Advanced coursework in alternative, complimentary, and lifestyle medicine makes her approach to health uniquely wholistic. Licensed, NYS medical cannabis practitioner.

Recipient of the Mohawk Valley 2010 Leader/Academic scholarship award Member of the International Honor Society of Nursing, Sigma Theta Tau Member of the American Nurses Association, NYS Nurse Practitioner Association.

 


ANITA S. FARRY, FNP-BCAnita Farry - HMABNY Provider

Anita has been in nursing for 40 years; 20 years as an RN, 20 years as a Family Nurse Practitioner. Practiced both in adult and pediatric settings.

Board Certified Family Nurse Practitioner by the American Nurses Credentialing Center with experience in community, acute and academic settings.

The last 2 years with Landmark as a longitudinal provider managing complex, chronically ill patients in the home setting. She is also an adjunct clinical instructor for SUNYPOLY. Member of Sigma Theta Tau-International Honor Society of Nursing and New York State Nurse Practitioner Association.

 


Craig Ritchie

Board Certified Adult Nurse Practitioner with a Master of Science Degree. 

Craig has provided primary care services to the elderly and disabled population doing homecare in patients’ private residences or assisted living homes since 2005 with  HMAA/HMAB, Landmark and the PACE  Program.

A recipient of the Outstanding Young Americans Award for Academic Achievement and Community Service. Honorable discharge from Army National Guard – Army Infantry, Reserves and ROTC.

Over 20 years of experience working as an Adult Nurse Practitioner in the Capitol District of New York. Wound care subject matter specialist. Preceptor for Nurse Practitioner students.

 


Chris Osterhoudt, MS, OTR/L, ECHM, CAPS

CHRIS OSTERHOUDT, MS, OTR/L, ECHM, CAPS

Chris is a Board-Certified Occupational Therapist; he received his Master’s degree in Occupational Therapy from Utica College and has been practicing since 2007. Chris has been providing Occupational Therapy services to adults from 18-106 yrs. of age, and has treated the acutely ill, chronically ill, elderly, and disabled individuals in hospitals and home-based settings, always with the goal of helping his clients build strength, reduce pain, and identify and implement strategies to increase independence with everyday task completion while reducing the risk of further injury.
Chris has served as an educator, teaching college coursework, and precepted graduate level Occupational Therapy students in various settings. Chris also achieved an Executive Certificate in Home Modification from the University Southern California and is a Certified Aging in Place Specialist through the National Association of Home Builders.
