
Kait is a board-certified physician assistant with over 15 years of clinical experience spanning nephrology, family medicine, and geriatrics. She brings a patient-centered and compassionate approach to care, with a strong foundation in chronic disease management and preventive medicine.
After earning her Bachelor of Science in Nutritional Science from the University of New Hampshire, Kait went on to complete her Master of Science in Physician Assistant Studies at the Philadelphia College of Osteopathic Medicine. Her background in nutrition has continued to inform her holistic and wellness-focused approach to patient care.
For the past eight years, Kait has served as a longitudinal provider at Landmark Health, delivering in-home care to medically complex and geriatric patients. In this role, she has been responsible for managing a wide range of chronic conditions, coordinating interdisciplinary care, and helping patients navigate the healthcare system to improve outcomes and reduce avoidable hospitalizations.

Chris is a Board-Certified Occupational Therapist; he received his Master’s degree in Occupational Therapy from Utica College and has been practicing since 2007. Chris has been providing Occupational Therapy services to adults from 18-106 yrs. of age, and has treated the acutely ill, chronically ill, elderly, and disabled individuals in hospitals and home-based settings, always with the goal of helping his clients build strength, reduce pain, and identify and implement strategies to increase independence with everyday task completion while reducing the risk of further injury.
Chris has served as an educator, teaching college coursework, and precepted graduate level Occupational Therapy students in various settings. Chris also achieved an Executive Certificate in Home Modification from the University Southern California and is a Certified Aging in Place Specialist through the National Association of Home Builders.
